Job Description

*Establishing and following a plan of care that identifies problems and evaluates outcomes through on-going assessments of the patient's response to treatments, medications, and education as needed.
*Providing direct care to terminally ill patients in the home setting, the hospital and the nursing homes.
*Giving one-on-one cares that allows you to have a real impact on outcomes.
*Treating your patients holistically, providing care and education.
*Enjoying independence and autonomy in your practice.

General Qualifications

Must have a current RN license with the state of Wisconsin. Must also have a valid driver's license from the state of Wisconsin, current auto insurance and a reliable vehicle.

Desired Qualifications

At least one year of acute care, medical/surgical or home care experience desired. This position will also do Home Care nursing visits. Independent assessment skills are necessary.
