Job: Respiratory Care Practitioner - Home Care Services

Ministry Home Care Services provides in home patients with Respiratory Therapy services to include:multiple options for oxygen systems for patients who need supplemental oxygen at home: oxygen concentrators, liquid oxygen systems, cylinder oxygen systems, oxygen conserving devices


These services are provided in a twenty two county area of North Central and North Eastern Wisconsin.


The incumbent will:

Render quality patient care in accordance with professional practice, standards, policies and procedures and accrediting body standards
Evaluate clients/patients for effectiveness of respiratory care, pulmonary management and specialized functions
Review physician orders, perform patient assessments and make appropriate clinical decisions regarding client/patient care
Communicate appropriate client/patient information to physicians and referral sources, clarifies written/verbal orders when necessary
Complete all required documentation, including client/patient assessments, progress notes, develop and update patient specific Plan of Care (POC) and discharge summaries, if applicable
Submit documentation in a timely manner, including HME Home Care Record documents and completed administrative forms
1.0 FTE (40 hours per week) 8:00 am - 5:30 pm Monday through Friday, on-call included

Education: Associate Degree in Respiratory Therapy program approved by the Council of Medical Education of the American Medical Association is required
Experience: 1 year of home care experience as a Respiratory Therapist is required
Experience: 1 year of acute care experience as a Respiratory Therapist is required
Licensure: Licensed as a Respiratory Care Practitioner in the State of Wisconsin is required
Certification: Basic Life Support (BLS) certification is required
Other: Current and vailid Wisconsin Driver's License is required
