Job: Physical Therapist - Neenah Home Health

POSITION SUMMARY:


Rehab professionals at Ministry Health Care put patients first and work with a multi-disciplinary team to provide comprehensive rehab services. Rehab professionals assist patients of all ages with varying functional limitations. This Home Health division offers the ability to create your own schedule - allowing excellent flexibility for those with busy lives. No weekends or holidays scheduled. Even if you have never thought about working in home health before, we can offer you an opportunity to do a ride-along visit with an experienced therapist, who can show the benefits of working in this area. In this position, you would also spend one day per week in a therapy coordinating role, where you would assist with overall management of services and ensuring that high quality care is given to our patients. This combination or dual role provides not only great flexibility but also variety throughout the work day. This position is full time, 36 hours per week, and includes full time benefits.



In this position, the incumbent will:


Travel to patient homes to implement and collaborate with the plan of care.


Understand and assure regulatory compliance issues.


Demonstrate efficient and effective use of available resources.


Serve as a role model and practice resource for agency staff.


Monitor and evaluates patient care outcomes.


Demonstrate expertise in the delivery of patient care.

Bachelor's degree in physical therapy from an accredited university

Minimum one year clinical experience preferred

Licensed as a physical therapist by the State of Wisconsin

CPR certification

Must have the ability to connect to the internet with company issued laptop from home using dialup or hi speed connection (DSL, cable, or satellite).

Valid driver's license, use of car and proof of auto insurance required
