Discharge Planning

Job Description

Imagine yourself

*Collaborating with patients and families to facilitate the plan of care.

*Collaborating with physicians daily to discuss patient progress, variances, and achievement of expected outcomes and plan of care.

*Facilitating communications between and negotiating with the patient/family, physicians, nurses, dietary, rehab, homecare, social services, and other disciplines that need to collaborate to provide care for the patient.

*Identifying, evaluating, and acting to resolve any potential barriers to delivery of care and a timely and appropriate discharge.

*Coordinating and documenting discharge plans of care.

General Qualifications

*Current licensure as a Registered Nurse (RN) granted by the State of Wisconsin.


Desired Qualifications

*Bachelor degree in Nursing preferred.

*Case management or utilization management experience preferred.
