Job Description:
To evaluate, plan, and provide treatment programs for patients receiving physical therapy. Provide a variety of health care services centered around the unique and individual needs of each patient according to the plan of care; meet the needs of all customers including patients, significant others, physicians, and other staff; work in a multidisciplinary team in order to deliver a full range of services and participate in team based continuous improvement. Perform patient evaluations, provide treatments, complete documentation, and supervise PTA's/Rehab Associates in a multi-disciplinary team in a homecare setting.


Skills / Requirements
Job Qualifications: Bachelors or Masters Degree in Physical Therapy as well as a valid WI physical therapy licensure required. Minimum of 2 years physical therapy experience preferred. Homecare experience preferred.
